This reporting tool was developed for clinicians to easily report safety concerns, and allow fingertip access to clinical protocols and educational material. The centerpiece of our efforts to improve communication and engagement is the Concern Reporting tool. It allows every member of our team to submit safety concerns in real time, without fear of retribution and to receive push notification of safety alerts to truly create a closed loop communication network. Two requirements of a reliability culture are the development of a mature reporting and learning culture. This app is a necessary tool for our journey to become a High Reliability Organization.
